Anti-Inflammatory Nutrition After Surgery

What it is
Inflammation is the body's response to surgical trauma, and it is one of the four phases wound repair runs through [1]. It is not the enemy. Suppress it hard enough and healing stalls. The interesting question is not how to shut it down but how it gets switched off cleanly once it has done its job, and there is a real biology to that.
Why it matters after a cosmetic procedure
Inflammation is what most of the visible recovery actually is. Swelling, bruising, tenderness and the pink of an early scar all trace back to it. That is why patients ask about it more than about anything else, and why the supplement market around it is as loud as it is.
The rest of this page is about which of those supplements have been tested in surgery and what the tests found. Some of the answers are less impressive than the marketing.
Bromelain
Bromelain is a mixture of protein-digesting enzymes from pineapple. Laboratory and animal work shows it has fibrinolytic, anti-swelling, antithrombotic and anti-inflammatory activity [2].
Most of the human trial evidence comes from dental surgery, specifically removal of impacted lower wisdom teeth. Across four separate meta-analyses of those trials, a reduction in post-operative pain is the finding that keeps appearing. The swelling result does not: two of the four found reduced swelling [3][4] and two found none [5][6].
The reason that matters is that swelling is what people take bromelain for, and the one randomized trial in facial plastic surgery tested exactly that. Forty-six rhinoplasty patients were randomized to bromelain or placebo, double-blind. Bruising and bleeding under the surface of the eye were significantly better in the bromelain group by day 7. Swelling showed no significant difference between the groups at any time point [7].
A 2026 systematic review of arnica and bromelain in facial plastic surgery found only two bromelain studies among the ten it could include, and concluded that the evidence for bromelain is limited [8]. An earlier review of these agents after cosmetic and laser procedures reached the same place: not enough data to support routine use [9].
So the honest summary is that bromelain has a plausible mechanism, a consistent signal on pain in dental surgery, and one facial-surgery trial where it helped bruising and did nothing for swelling.
Quercetin
Quercetin is a flavonoid found in onions, apples and berries. It inhibits histamine release and reduces production of several inflammatory signaling molecules [10].
There is a caveat that belongs right next to that, because it is the sort of thing that gets left out. Quercetin lowers the expression of an inflammatory enzyme in human cells in the laboratory, but when that was tested in living people, a single high dose from onions did not change it [11]. Cell-culture activity and an effect in a person are different claims, and only the first one is established here.
Omega-3 fats
EPA and DHA are the starting material for a family of molecules including resolvins, protectins and maresins, collectively called specialized pro-resolving mediators. These are not blockers. They are the signals that actively bring an inflammatory response to a close [12]. That is a distinct idea from anti-inflammatory in the usual sense, and it fits the surgical situation well. The goal is not less inflammation. It is inflammation that ends on time.
Many surgeons ask patients to stop fish oil and other blood-thinning supplements before surgery; your surgeon's list governs. It is worth knowing that this advice has been directly tested. In 1,516 patients randomized before cardiac surgery, fish oil did not increase perioperative bleeding, and the fish oil group needed fewer units of transfused blood. Patients with the highest blood omega-3 levels on the morning of surgery had the lowest bleeding risk, and the authors called for reconsideration of the standing advice to stop fish oil before cardiac procedures [13]. That trial was in cardiac surgery at a high dose, so it does not settle the question for a facelift. It also does not override the instruction you were given. It does mean the question is live rather than closed.
Curcumin
Curcumin is the active compound in turmeric. It suppresses activation of NF-kB, a master switch for inflammatory gene expression, at a step before the switch is thrown [14].
In humans, a double-blind randomized trial in 50 patients having their gallbladder removed found lower pain scores and lower fatigue scores in the curcumin group across the first three weeks, and far lower use of pain tablets [15]. Note what was measured there: pain and fatigue, reported by the patient. Swelling was not an endpoint, and no inflammatory marker was measured.
Curcumin on its own is poorly absorbed, which is why bioavailability-enhanced forms exist. In human volunteers, curcumin alone produced blood levels that were undetectable or very low, and adding piperine raised them sharply over the first hour after the dose [16].
Food
Dietary patterns associated with lower inflammation lean toward colorful vegetables, berries, oily fish, olive oil, ginger, and green tea, and away from refined sugar, heavily processed food, and alcohol. Adequate fluid intake supports the lymphatic system, which is what clears the debris of an inflammatory response out of a healing area.
That is a description of a pattern, not a prescription. What you should eat and drink after your operation, and when, are part of your post-operative instructions.
Timing
Nothing on this page comes with a start date. The window before surgery belongs entirely to your surgeon, and several of the compounds discussed here are on the list of things surgeons commonly ask patients to stop beforehand. That is the conversation to have, and the pre-operative visit is where to have it.
Bottom line
The mechanisms behind anti-inflammatory nutrition are real and in several cases well characterized. The human trial evidence in cosmetic surgery is much thinner than the category's reputation, and where it exists it is more specific than the marketing suggests: bromelain helped bruising and not swelling in the one rhinoplasty trial that tested it. Knowing that is more useful than a longer list of supplements.
