LEAN uses GLP-1 receptor agonist therapy to help regulate appetite signals. Your prescribing provider selects the medication and the form based on your clinical profile, medical history and goals.
Dosing begins low and titrates upward under clinical guidance. You will have regular check-ins with your provider to monitor progress, adjust dosing and manage side effects. The medications dispensed in this program are compounded formulations, which the FDA has not evaluated for safety or effectiveness.

Compounded and off-label. The medications in this program are compounded formulations prepared by FDA-registered pharmacies. The FDA does not review compounded drugs for safety, effectiveness or quality before they are marketed, and the trials below studied branded products rather than what is dispensed here. In SURMOUNT-1, adults with obesity or overweight and without type 2 diabetes who received branded injectable tirzepatide lost a mean of 15.0 percent of body weight at 5 milligrams, 19.5 percent at 10 milligrams and 20.9 percent at 15 milligrams over 72 weeks, compared with 3.1 percent for placebo (Jastreboff et al., New England Journal of Medicine, 2022). In STEP-1, branded injectable semaglutide at 2.4 milligrams weekly produced a mean reduction of 14.9 percent over 68 weeks, compared with 2.4 percent for placebo (Wilding et al., New England Journal of Medicine, 2021). In OASIS-4, branded oral semaglutide at 25 milligrams daily produced a mean reduction of 13.6 percent at week 64, compared with 2.2 percent for placebo (Wharton et al., New England Journal of Medicine, 2025).

What is the difference between Tirzepatide and Semaglutide?
Tirzepatide acts on both the GIP and GLP-1 receptors, while semaglutide acts on the GLP-1 receptor. Semaglutide is offered in this program as a weekly injection or as a daily oral dose. All three are dispensed as compounded formulations, which the FDA has not evaluated for safety or effectiveness. Your provider selects the medication and the form based on your clinical profile.
Is this FDA-approved?
No. These are compounded formulations. The FDA does not review compounded drugs for safety, effectiveness or quality before they are marketed, and it has stated that some compounded semaglutide products use salt forms that are different active ingredients from those in the approved drugs. How quickly will I see results?
Individual response varies, and your provider is the right person to ask about your own expectations. The published trials cited on this page measured body weight at fixed endpoints, 72 weeks in SURMOUNT-1 and 68 weeks in STEP-1. They studied branded products rather than the compounded formulation dispensed here.
Are there common side effects?
The FDA labels for the branded GLP-1 medications list nausea, diarrhea, vomiting, constipation and abdominal pain among the most common adverse reactions. Your provider screens at intake for contraindications, including a personal or family history of medullary thyroid carcinoma or MEN2 and prior serious hypersensitivity to the medication, and for warnings, including a history of pancreatitis.
Do I need to follow a special diet?
No specific diet is required, but your provider may recommend nutritional guidelines to maximize results and support overall health.
Can I stop the medication once I reach my goal weight?
Your provider will work with you on a plan for maintenance. Discontinuation should always be done under clinical guidance.
How is the medication administered?
